SDMHA Registration for 2011-12 Season – Mahg to Midget
Registration for this coming hockey season is CLOSED! Deadline was July 26, 2011. If you have missed the deadline and would like to inquire, please contact our Registrar, Tracy Huckabone at tracy.huckabone@live.com.
Please mail your completed e-reg form 2010 (one per child) to the following address:

- Or contact SDMHA Registrar to arrange drop off
Registration fee for 2011-12 season – $360 (Novice to Midget), $295 (Mahg)
If your child is interested in registering for the CC conditioning camps, (Atom to Midget), an additional fee of $50 will also be due at the time of registration.
Full payment MUST accompany registration forms (post-dated cheques up to December 1, 2011 are accepted). Registration and CC conditioning camp fees MUST be paid before players will be allowed on the ice. Please contact SDMHA Registrar if you have any questions about payment plans.
